29 CFR 1910.178(l)(5): A powered industrial truck operator who had received training in the topics specified in paragraph (l)(3) of this section appropriate for the truck and working conditions encountered from a previous employer was not evaluated and found competent to operate the truck safely by their current employer: (a) Location - 470 Wildwood Avenue #5, Woburn, MA: On or about 5/4/2018, the operator of the Big Joe Model PDM-30-130 Serial #378000 Class III Powered Industrial Truck, who had received training from previous employers, was not evaluated and found competent to operate the truck safely before being permitted to operate the truck.
