29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    (a) On or about 5/3/2018 - at 2341 Crossplex Blvd, Birmingham, AL 35208, eye protection was not used when employees were spraying A44W50, TUFF SURFACE Premium Texture Coating with a Graco Texspray RTX 1500 machine then knocking down the texture on the walls with hand tools, such as but not limited to, a squeegee exposing employees to liquid chemicals.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:    (a) On or about 5/2/2018 - at 2341 Crossplex Blvd, Birmingham, AL 35208, employees were not protected from falling approximately 33 feet when the guardrail system at the 4th floor wall opening was removed.
