29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about May 1, 2018 at 16 Willow Oak Lane, Fort Mitchell, Al: The employer did not ensure employees installing shingles on back side a residential roof with a 4:12 pitch were protected from falls to the ground of 9 feet 9 inches.  b) On or about May 1, 2018 at 16 Willow Oak Lane, Fort Mitchell, Al: The employer did not ensure an employee installing shingles on side of a residential roof with a 10:12 pitch was protected from falls to the ground of 18 feet 7 inches.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:  a) On or about May 1, 2018 at 16 Willow Oak Lane, Fort Mitchell, Al: The employer did not ensure employees installing shingles on a 4:12 pitch residential roof used proper anchors in that their rope was doubled over in a loop with two 2x4 boards nailed over the rope and then a couple of bundles of shingles placed onto the 2x4's which exposed them to potential falls of 9 feet 9 inches to the ground.  b) On or about May 1, 2018 at 16 Willow Oak Lane, Fort Mitchell, Al: The employer did not ensure an employee installing shingles on a 10:12 pitch residential roof used proper anchors in that their rope was doubled over in a loop with two 2x4 boards nailed over the rope and then a couple of bundles of shingles placed onto the 2x4's which exposed them to potential falls of 18 feet 7 inches to the ground.
