29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: (a) On or about 5/2/2018 - at 2341 Crossplex Blvd, Birmingham, AL 35208, employees were not protected from falling approximately 33 feet when the top guardrail at the 4th floor wall opening was removed and employees were removing doors from pallets that were elevated on a pallet by a Genie GTH1056 telehandler. (b) On or about 5/2/2018 - at 2341 Crossplex Blvd, Birmingham, AL 35208, employees were not protected from falling approximately 33 feet when the guardrail system at the 4th floor wall opening was removed and employees were positioning a Graco Texspray RTX 1500 machine at the wall opening and then maneuvering the machine from the building to the job-built trash box or scaffold work platform that was elevated by a Genie GTH1056 telehandler.
