(i) When work was performed less than 6 feet (1.6 m) from the roof edge, the employer did not ensure each employee was protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system: On or about May 7, 2018 at a worksite located at 5915 Canal Road, Valley View, OH 44125: during roof repair and maintenance activities on a low slope roof employees were exposed to fall hazards of approximately 11.5 to 13.9 feet. The employer failed to ensure fall protection systems/devices were used by all employees working on the roof.

29 CFR 1910.30(a)(1): Before any employee is exposed to a fall hazard, the employer did not provide training for each employee who uses personal fall protection systems or who was required to be trained as specified elsewhere in this subpart: On or about May 7, 2018 at a worksite located at 5915 Canal Road, Valley View, OH 44125: during roof repair and maintenance activities on a low slope roof employees were exposed to fall hazards of approximately 11.5 to 13.9 feet. The employer failed to ensure all employees working on the roof exposed to fall hazards were properly trained.
