The Superintendent operating the CAT TL1055C Telehandler/Rough Terrain Forklift and who was lifting framing materials to the upper levels of the structure had not had his performance evaluated on the Rough Terrain Forklift in over 3 years. Violation observed on or about 05/04/2018.
