29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Location: 67 Lefferts Ave, roof top  on or about: May 7 2018  a) Employee exposed to fall of approximately 30 feet. Employee was working on a sloped roof installing a roll of fabric and was not wearing any fall protection.

Employee worked on a sloped roof and exposed to a fall of approximately 30 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed  Location: 67 Lefferts Ave, roof top  On or about: May 07 2018                  a) Employees were using a 30 foot extension ladder to access the roof of a two story residence. The ladder did not extend 3 feet past the intended upper landing.
