29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: Spa Roof - On or about May 3, 2018, employees were exposed to fall hazard while jumping on to and off a 48-inch lift of Hi-def board without the use of a ladder, stairway or other safe means.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function: (Construction Reference 1926.103) a) Encore Casino, Basement Level 1, 38C, Horizon Drive, Everett, MA. On or about May 3, 2018, the employer did not ensure that employees wearing tight-fitting respirators were free of facial hair that came between the face and the respirator. Employees were found to have facial hair that interfered with the seal of the respirator. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
