On or about May 7, 2018, employees where on a pump jack scaffold at a height greater than ten feet above lower level and the poles were not braced on the bottom to prevent being displaced.

An employee was leaning out a second floor window at a height greater than ten feet above level with no fall protection system in place to prevent him from falling. Instance 2. Thirteen windows (on second floor) had no railings in place to protect employees from falling to lower level which was greater than ten feet to lower level.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge. Location - 170 Erie Street, Cambridge, MA 02139. Inside of the house towards the middle and outside back of the house. No or about May 7, 2018. Instance 1. employees were exposed to falling when ascending and descending the house from the back door which was being used as the main entrance. The stairs had no railing system in place to prevent them from falling. Instance 2. Employees were exposed to falling when ascending and descending the first and second using stairs that had no railings in place to prevent them from falling.
