29 CFR 1910.178(l)(1)(i): Operators were not trained in the safe operation of powered industrial trucks as demonstrated by the successful completion of the training and evaluation specified in this paragraph.   a) Dock Area: On or about May 1, 2017, and at times prior, the employer exposed employees to struck- by hazards in that the employer failed to train forklift drivers in use of the Toyota forklift (Model #5FBFI8; S/N 26365).  b) Warehouse/ Dock Area: On or about May 1, 2017, and at times prior, the employer exposed employees to struck- by hazards in that the employer failed to train forklift drivers in use of the Pallet Pro pallet jack (S/N CCG1181).

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain at each workplace, a written hazard communication program which described how the criteria specified in 29 CFR 1910.1200 paragraphs (f), (g), and (h) for labels and other forms of warning, safety data sheets, and employee information and training would be met.   a) Production Area: On or about May 1, 2018, and at times prior, the employer exposed employees to fire, skin and respiratory irritation hazards in that a written hazard communication program was not provided to them while using chemicals such as but not limited to U-Line Silicone Lubricant S-19036 and Calcium Lime Rust Remover.
