B.) Scobell Company, Inc., on or about March 19, 2018 and times prior thereto: The employer's form used to record an injury that occurred on March 19, 2018 was not equivalent to OSHA Form 300 in that information from columns A, C, E, G-J, and M were not included. In addition, needed information as prescribed by column F of OSHA Form 300 was not included on the employers form in that the form did not include information about the object/substance that directly injured or made the person ill. Abatement Certification and Documentation is required.
