29 CFR 1926.453(b)(2)(v): A lanyard was not attached to the boom or basket when working from an aerial lift:    a)  416 N. Second Street, Bainbridge PA - An employee was observed working from a JLG aerial lift and did not have the lanyard of a personal fall arrest system attached to the boom or basket, exposed to an approximate 15 foot fall hazard to the ground below, on or about May 7, 2018.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a)  416 N. Second Street, Bainbridge PA -  An employee was observed installing siding while working from a flat roof and was not protected from an 11 foot - 4 inch fall hazard to the ground below, on or about May 7, 2018.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder:    a)  416 N. Second Street, Bainbridge PA - Employees were observed using a portable Louisville fiberglass extension ladder to access the roof and the ladder was set up at an unsafe angle, on or about May 7, 2018.  The horizontal distance from the top support to the foot of the ladder exceeded one-fourth of the working length.  The foot of the ladder measured 87 inches from the top support and should have measured no more than 34 inches.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
