29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer is failing ensure that employees are protected from amputation hazards. This was most recently documented at 302 Jeffers Avenue Lyman, Nebraska 69352 on April 25, 2018. The employer failed to ensure that machine guarding was provided on the Torrington punch press. An employer reached into machine to try to a part. The machine actuated, causing an amputation of the employees finger.

This was most recently documented at 302 Jeffers Avenue Lyman, Nebraska 69352 on April 25, 2018. An electrical box, which was intended for use while permanently fixed to a wall and which had pre-punched knockouts, was attached to the end of a flexible cord and was used to power a reamer.
