29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. Casino Warehouse Rooftop: Strain relief was not provided to a white extension cord, the outer sheathing of the extension cord was pulled back exposing the inner conductors to physical damage. The cord energized a DeWALT electric drill.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: Casino Warehouse Rooftop : Employees were not provided with fall protection while engaged in roofing work around skylights. The employees were exposed to 25 and 15 foot falls to lower levels.
