29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user shall request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered. a) Warehouse: On or about May 8, 2017, and at times prior, the employer exposed employees to struck-by and crushing hazards when operating Forklift (Mitsubishi Model FGC30NQ Serial Number AF83F00676), in that the forklift had a an attachment but its data plate did not reflect the new lifting capacity of the forklift.
