29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  On or about April 26, 2018, an employee was exposed to a fall hazard greater than 10 feet to the dirt ground below while working on a tier of a tubular framed scaffold which was not fully planked.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about April 26, 2018, an employee was exposed to a fall hazard greater than 6 feet to the dirt ground below while using a piece of 2x4 board as a bridge to cross from one awning to the next.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about April 26, 2018, an employee was exposed to a fall hazard to the dirt ground below while using a multipurpose ladder that was not extended at least 3 feet above the upper landing surface being accessed.
