29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee Location: Jobsite On or about May 1st 2018, an employee was exposed to a 16 foot fall hazard while working in a stairway where the fall arrest system was attached to an anchorage point being used to hoist a steel stairway into place.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Location: East Façade, Convention area, section M On or about May 3. 2018, the employer did not ensure that the main electrical power cord to the Skyjack, model #SHJIII3219 was protected from strain relief and exposed the internal electrical wires.
