29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):     (a)Crestone Builders, Inc. at 2001 East Kemp Place, Watertown, SD 57201: On or about and at times prior to May 8, 2018, the exposing employer did not ensure all forklift operators were trained in the operation of powered industrial trucks in that, the operators were not trained in the use of the Genie GTH-1056 or the JLG /Gradall G6-42A Rough Terrain forklifts prior to use. This condition exposed the employees to the hazard of improper forklift operation.     Abatement Note: Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance.

Employees were exposed to a tripping hazard due to multiple open holes approximately 4 inches by 4 inches in place for pipe installations. This condition exposed employees to trip hazards as it was located along a common path of travel.
