29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables were used as a substitute for fixed wiring of a structure: Meat Department: An orange extension cord was used in lieu of permanent wiring to energize a Bulker meat grinder. Employees were exposed to electrical shock hazards. The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1012535, citation number 01, item number 005 and was affirmed as a final order on 04/10/2015, with respect to a workplace located at Moffett Field Bldg. 12, N Akron Road in Mountain View, CA 94035.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to the joints or terminal screws: Meat Department: The Bulker meat grinding machine power supply cord lacked strain relief on its plug end into the machine. The outer sheathing of the power cord was pulled back exposing employees to electrical hazards. The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 124277, citation number 02, item number 001 and was affirmed as a final order on 01/08/2018, with respect to a workplace located at Bldg. 02023 NAWS China Lake in Ridgecrest, CA 93555. The Defense Commissary Agency was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1140641, citation number 02, item number 00 and was affirmed as a final order on 06/07/2016, with respect to a workplace located at 795 Reeves Blvd. in Lemoore Naval Air Station, CA 93246.
