29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: Hotel 22nd floor On or about May 1, 2018 a flexible extension plug was pulled away from the outer protective casing exposing the conductor wires.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: 6th floor hotel, E stairwell On or about May 3, 2018, employees using temporary horizontal lifelines were not trained in the manufacturer's installation instructions.
