29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) On or about April 18, 2018, and at times prior, the employer did not ensure that employees working on a roof were protected from falls where they raised and lowered workers to and from the working platform with a modified attachment on an articulating boom lift that was not designed for personnel use and did not have guardrails or anchors for fall restraint. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a)  On or about April 18, 2018, the employer did not ensure that employees working on a three story roof were protected from falls where they did not have appropriate fall arrest systems, ropes, or anchors in place. 29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body narness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  a) On or about April 18, the employer did not ensure that employees working on a three story roof were using appropriate fall protection designed in accordance with OSHA requirements, where they were wearing rock climbing harnesses that attach to rope through loops in the front at the user's waist.
