29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge: (a) This violation occurred on May 8, 2018 at the emergency exit door on the South West wall at the front of the building; which was latched and locked, exposing employees to a burn hazard in the event of a fire. (b) This violation occurred on May 8, 2018 at the emergency exit door on the North East wall at the back of the building; which had a very heavy bar placed a crossed it from the inside, exposing employees to a burn hazard in the event of a fire.

1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) This violation occurred on May 8, 2018 when the employer did not ensure that twenty-six of twenty-eight employees required to operate Crown forklifts and-or powered pallet jacks were trained to do so; exposing employees working within the warehouse to a struck-by hazard.
