29 CFR 1926.28(a): The employer is responsible for requiring the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees.  On or about May 9, 2018 at 2146 Elwood Ct., Baton Rouge, La. 70809; the employer allowed employees to toss bricks while working from scaffolding without wearing any type of head protection.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access.  On or about May 9, 2018, 2146 Elwood Ct Baton Rouge, La. 70819; the employer allowed employees to work from a scaffold that did not have a ladder as a safe means of access or egress.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about May 5, 2018, at 2146 Elwood Ct, Baton Rouge, La. 70809; the employer allowed employees to access the second story roof of a residential home from a scaffold, while not wearing fall protection or protected by a complete guard rail system on each side of scaffold.
