29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse a) Due to stacking and storing of heavy plumbing supply (ie pipes, radiators) warehouse personnel were exposed storage rack collapse hazard and falling object (pipes) hazard: --- instance 1) several storage racks were heavily loaded when the employer had not determined the weight of the load to ensure it does not exceed the rated capacity of the racks; --- instance 2) storage cages were stacked; some cages were bent, damaged; --- instance 3) steel pipes on stacks were left unsecured; can roll off onto employees; --- instance 4) steel radiators were stacked unsecured; can slide off onto employees; located outside the loading dock; Location: 471 Sutter Ave, Brooklyn, N.Y.
