29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:  On or about May 9, 2018, in the clean rooms and in the poly room, employees used chemicals  containing organic vapors, such as but not limited to, xylene and acetone to process and make resistors for electronic components. The employer did not conduct air monitoring to see if employees were over permissible exposure limits for time weighted average.

29 CFR 1904.41(a)(2): 29 CFR 1904.41(a)(2): Annual electronic submission of part 1904 records by establishments with 20 or more employees, but less than 250 employee. The establishment had 20 or more employees at some time during the previous calendar year, and this part required the establishment to keep records, but the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. The employer did not submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form:  On or about May 9, 2018, at this location, the employer did not electronically submit OSHA 300A log information for 2016.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  On or about May 10, 2018, at this establishment in the poly room, an employee was voluntarily wearing a dust mask when processing resistor plates. The employer did not furnish to the employee all information found in Appendix D to 29 CRF 1910.134.
