29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  (a) Accent 2050 Apartments, North side elevation: On or about 05/09/2018, and at times prior, the employer exposed an employee to ejection/fall hazards of approximately 50-feet to the lower ground level, in that a lanyard was not provided to and employee wearing a full body harness installing exterior sheathing from a JLG Aerial Lift. The employee was not connected to marked locations on the basket.
