29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: (a) On or about May 09, 2018 - South Dock, Birmingham Annex, 4500 1st Avenue South, Birmingham, AL, forklift operator's performance had not been evaluated since 03/19/2015. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(l)(4)(iii), which was contained in OSHA Inspection number 1266758, citation number 1, item number 1 and was affirmed as a final order on 03/01/2018, with respect to a workplace located at 351 N 24th Street, Birmingham, AL 35203.
