29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) Production Area- On or about May 8, 2018, the employer failed to establish a Lockout Tagout Program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on machines, such as the Towel Folding Machines, Wash Line Machines, where the unexpected energization, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  b) Wash Line- On or about May 8, 2018, the employer failed to establish a Lockout Tagout Program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performing unjamming of machines, such as the Wash Line Machines, where the unexpected energization, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points and rotating parts:    a) Wash Line- On or about May 8, 2018, the employer failed to ensure that hazards created by the automated moving shuttle on the Wash Line were guarded to prevent injury to employees while removing fallen towels and while walking through the area.    b) Wash Line- On or about May 10, 2018, the employer failed to ensure that hazards created by the pinch point of the automated closing of the Wash Line Dryers 1, 2, and 3 were guarded to prevent injury to employees during the removal of linens from the dryers.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:    a) Wash Line- On or about May 10, 2018, the employer did ensure that all unused openings on electrical boxes were effectively closed on the 480 Volt Wash Line Control Panel.     No abatement certification or documentation is required for this item.
