29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices.    a) North Building Production Area - On or about May 2, 2018, the employer failed to provide adequate training for employees performing service and/or maintenance activities to ensure the knowledge and skills required for the safe application, usage and removal of energy control devices were acquired.  A laborer/saw operator sustained a severe laceration to the back of the left hand while attempting to clear wood from near the sawblade of an operating Newman KM16 Trim Saw.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)(6):   a) North Building Production Area - On or about May 2, 2018, employees were exposed to machine hazards associated with moving parts when clearing wood from near the sawblade of the Newman KM16 Trim Saw. The employer failed to implement energy control application steps as the machine was not shut down or turn off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)((i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:    a) North Building Production Area - On or about May 10, 2018, the employer failed to ensure that each of the three employees involved in the servicing task of changing the sawblade(s) on the Newman KM16 Trim Saw affixed a personal lockout device (lock) onto a group lockout device. Only one lock was used to lockout the trim saw, thereby the two other employees assisting with changing the sawblade(s) were exposed to hazards associated with the unintentional startup of the saw.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:    a) North Building Production Area - On or about May 10, 2018, authorized employees were required to perform servicing and/or maintenance on the Newman KM16 Trim Saw, Whirlwind Model 212 Cutoff Saw, Hazelthorn A10 Single Notcher and other operating equipment. The employer did not ensure periodic inspections of the energy control procedures were performed at least annually as required by the standard.
