29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a) Sellan Steel LLC at 2080 California Street in Denver, CO 80205:  On or before May 8, 2018, the exposing employer did not ensure that a fall protection system was in place for each employee working 10 feet or higher above the ground on a scaffold. An employee of Sellan Steel was dismantling a scaffold on the 11th floor of a construction site without the use of a fall protection system. This condition resulted in exposure a fall hazard of approximately 100 feet.

29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:    a)  Sellan Steel LLC at 2080 California Street in Denver, CO 80205:  On or before May 8, 2018, the exposing employer did not ensure that employees were standing on the floor of the basket while performing work.  An employee was performing welding work while standing on the midrail of the basket approximately 80 feet above the ground.  This condition exposed this employee to fall hazards caused by working on a surface that was not designed to be worked on.

29 CFR 1926.454(b): The employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.    a) Sellan Steel LLC at 2080 California Street in Denver, CO 80205:  On or before May 8, 2018, the exposing employer did not have each employee involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question.  Employees who were dismantling scaffolding 11 stories above ground were not adequately trained to accomplish this work safely.  An employee was standing on the midrail of an aerial lift and was not adequately trained to accomplish this work safely.  This condition resulted in exposure to fall hazards caused by inadequate training.

29 CFR 1926.451(h)(2): Employees working below a scaffold were not protected from falling tools, materials or equipment in accordance with the provisions of paragraphs (h)(2)(i)-(v) of this section:    a) Sellan Steel LLC at 2080 California Street in Denver, CO 80205:  On or before May 8, 2018, the exposing employer did not ensure that employees working below a scaffold were protected from falling tools, materials or equipment in accordance with the provisions of paragraphs (h)(2)(i)-(v) of this section.  Employees were disassembling the Hydro Mobile lift scaffold platform at  the east side of the building, and employees walking/working below the scaffold were not protected from falling objects.  Objects such as water bottles and nails fell approximately 100 feet from the scaffold as it was being disassembled on the 11th floor.  This condition exposed employees to the hazard of being struck-by falling objects.

29 CFR 1926.51(a)(1): An adequate supply of potable water was not provided in all places of employment:  a) Sellan Steel LLC at 2080 California Street in Denver, CO 80205:  On or before May 8, 2018, the exposing employer did not provide potable drinking water to their employees. This condition exposed employees to heat exhaustion hazards due to dehydration.  Potable water means water that meets the standards for drinking purposes of the State or local authority having jurisdiction, or water that meets the quality standards prescribed by the U.S. Environmental Protection Agency's National Primary Drinking Water Regulations. (40 CFR part 141).

29 CFR 1926.95(d)(1): Except as provided by paragraphs (d)(2) through (d)(6) of this section, the protective equipment, including personal protective equipment (PPE), used to comply with this part, was not provided by the employer at no cost to employees.  a) Sellan Steel LLC at 2080 California Street in Denver, CO 80205:  On or before May 8, 2018, the exposing employer did not provide personal protective equipment to employees.  Employees were supposed to wear safety glasses, hard hats, and personal fall arrest system equipment when working onsite.  The employer did not pay for this equipment, and employees had to pay for their personal protective equipment.
