On or about May 10, 2018: Employees making acid baths for the etching process are required to wear a North 7700 Half Face Respirator with cartridges.  The employer did not ensure the employees were medically evaluated.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.    On or about May 10, 2018: Employees working in the etching room and the dark room are exposed to corrosive chemicals such as but not limited to nitric acid, ferric acid, Hydro-Coat Cold Developer Concentrate, and Rapid Assess Hard Dot Fixer Concentrate (acetic acid).  The rooms do not have an emergency eyewash station.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.   On or about May 10, 2018: Employees working in the production area are exposed to trip hazards as well as fire hazards where the fire extinguisher near the water jet cutter was completely blocked by a clicker press and boxes.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment.    On or about Mary 10, 2018: Employees working in the facility are exposed to trip hazards and electrical related hazards where the electrical panel boxes are blocked.   (a) In the electrical room the electrical panel to the right, Disconnect 480/277 volts, blocked by water jugs, equipment and boxes.  (b) The CNC electrical panel box, 480 volts, slightly blocked by a drum sander.  (c) The main air compressor electrical panel box, blocked by cardboard and wood.  (d) Electrical panel A, from 9 kva transformer, 400 Volts blocked by a shop vac and air hose.    (e) Electrical panel near the laser machine was blocked by cylinders, a chair, and other items.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical.   On or about May 10, 2018: Employees working in the production area are exposed to chemicals such as but not limited to nitric acid, ferric acid, ethanol, degreasers, Hydro-Coat Cold Developer Concentrate, and Rapid Assess Hard Dot Fixer Concentrate (acetic acid). Some containers within the production areas did not have pictograms and other identifying words and hazard warnings.

On or about May 10, 2018: Employees working in the production area are exposed to smoke and fire hazards as well as other injuries in the event of an emergency where doors that are not exits could be mistaken as such.  The employer did not ensure that these doors had the proper signs.
