29 CFR 1926.502(b)(1): Top edge height of top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: a) Job site, 6451 Stuebenville Pike Road, Pittsburgh, PA: On or about May 8, 2018, during carpentry and other construction activities, the employer did not ensure that appropriate perimeter wall structures were of adequate height to serve as a guardrail system in that parapet wall heights ranged from approximately 18 inches tall to 29 inches tall, exposing employees to falls of up to approximately 16 feet.

29 CFR 1910.134(k)(6): The basic advisory information on respirators, as presented in Appendix D of this section, was not provided by the employer in any written or oral format, to employees who wore respirators when such use was not required by this section or by the employer: a) Job site, 6451 Stuebenville Pike Road, Pittsburgh, PA: On or about May 9, 2018, during carpentry and other construction activities, the employer did not ensure that employees were provided with the information contained in Appendix D of this section where employees use of dust masks or N95 filtering face piece type respirators were used on a voluntary basis.
