On or about May 10, 2018, at the above addressed jobsite, an employee was exposed to falls while performing glazing operations from a tubular welded frame scaffold, approximately sixteen (16) feet seven (7) inches above the ground, that did not have the working level fully planked.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. a. On or about May 10, 2018, at the above addressed jobsite, an employee conducting glazing operations from a tubular welded scaffold, sixteen (16) feet seven (7) inches above the ground, did not have the vertical ladders on the scaffold lined up, thereby exposing the employee to falls.
