29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: a) Job site, 6541 Steubenville Pike Road, Pittsburgh, PA: On or about May 9, 2018, during water line installation activities, the employer did not ensure that a safe means of egress was provided for an employee in an excavation determined to be over 4 feet in depth in that the employee had to climb under/over trench box shoring supports to reach the ladder, which was a step ladder folded up and leaned against the excavation wall, that was missing appropriate legible labeling and that had a badly damaged top which affected the overall structural integrity of the ladder.
