29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: a) Throughout the establishment: On or about May 10, 2018, and at times prior, the employer exposed employees to amputation hazards, in that a periodic inspection of the lockout tagout program was not performed for employees working on machines, such as but not limited to, conveyor belts and BFB Wrapper machines.

The employer contacted OSHA on May 2, 2018.
