29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:  On or about May 3, 2018 at 3936 LaVista Road, Tucker, Georgia, the employer did not ensure that employees performing electrical repairs received training in the safety-related work practices such as but not limited to verification that electrical defects and/or hazards were eliminated after repairs were made.  An employee was exposed to live electrical parts after opening the signs frame to install light bulbs.

On or about May 11 at 3936 LaVista Road, Tucker, Georgia, the employer failed to train the employees on the use of safety related work practices such as, working on or near live parts, de-energizing and testing equipment and LOTO requirements after equipment is de-energized.

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were de-energized, the circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2):   On or about May 3, 2018 at 3936 Lavista Road, Tucker, Georgia, the employees making repairs to the sign turned off the circuit, but did not lock and tag out the circuit.

29 CFR 1910.333(b)(2)(iv)(B):  Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that the circuit elements and equipment parts were deenergized:  On or about May 3, 2018 at 3936 LaVista Road, Tucker, Georgia after replacing the ballast and prior to replacing lamps in the sign, a qualified person did not use testing equipment to verify that the wall mounted disconnect actually deenergized the sign.
