29 CFR 1910.67(c)(2)(v): (v) A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, shall be worn and attached to the boom or basket when working from an aerial lift.      (a)   Power pole 55F2687 in front of Little Caesars Pizza - On or about May 10, 2018 an employee was exposed to a fall hazard while working inside a Ford F550 Super Duty bucket truck without fall protection.  This employee was elevated at 22 feet above the ground.

(a) Power pole 55F2687 in front of Little Caesars Pizza - On or about May 10, 2018 an employee was exposed to a struck-by hazard while walking under a bucket truck basket with no hardhat.
