29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)  Ground level of the construction site - On or about May 11, 2018 employees were exposed to a struck-by hazard due shingles being shoveled off the roof. None of these employees were wearing hardhats.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a)  Roof - On or about May 11, 2018 employees were exposed to a flying object hazard while using nail guns due to the fact they were not wearing eye protection.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  (a)  Roof - On or about May 11, 2018 employees were exposed to a fall hazard while working on a steep roof 27 feet above the ground with no fall protection.
