29 CFR 1926.453(a)(2): The employer did not ensure "field modified" aerial lifts for uses other than those intended by the manufacturer, obtained a certification for the modification, in writing by the manufacturer or by any other equivalent entity, such as a nationally recognized testing laboratory, to be in conformity with all applicable provisions of ANSI A92.2-1969 and this section and to be at least as safe as the equipment was before modification: On or about May 11, 2018, at a worksite located at 1 MGM Way, Springfield, MA, the employer did not obtain a written certification from the manufacture or equivalent entity, before field modifying a JLG Boom Lift basket. The employer had employees build an extending supporting arm from the basket, made from 2"X4" pieces of wood to lift material.

29 CFR 1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section: One MGM Way, Springfield MA: On or about 5/11/18, employees in lifts were cutting glass fiber reinforced concrete (GFRC) with handheld power saws, and the employer did not ensure saws were equipped with a commercially available dust collection system at the point of operation to minimize and control release of silica dust as required by Table 1.
