29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a) JD Roofing LLC located at 1169 Picard Lane, Fort Collins, CO 80526: On and preceding 5/14/2018, the employer did not ensure that employees were protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects. Employees working on different levels of a residential roof were not utilizing head protection. The employees were installing shingles and handing other materials on the two levels of the roof which put one employee holding materials above the other employee without head protection utilized. This exposed the employees to struck by hazards.

Employees were using and extension to access the roof with approximately 1' extending above the landing surface. This exposed employees to falls from height.
