One employee sheathing the roof was grabbing panels of plywood through trusses and securing them in place without the use of fall protection. The employee did not have a harness and anchors were not installed.  This exposed the employee to falls from height.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:    (a)  Endos Construction located at 6181 Eagle Roost Drive, Fort Collins, Co 80524:  The employer did not ensure that   stairways having four or more risers were equipped with a stairrail system along each unprotected side or edge  employees engaged in residential construction used the stair located in the garage to access the interior of the home. The temporary staircase in the garage had 6 risers and both side were not protected by a stairrail.  This exposed employees to falls from height.    (b) Endos Construction located at 6181 Eagle Roost Drive, Fort Collins, Co 80524:  The employer did not ensure that   stairways having four or more risers were equipped with a stairrail system along each unprotected side or edge  employees engaged in residential construction used the stairs in the main living area to access the second floor of the home while carrying materials and tools. The staircase in the main living area had 18 risers with one side not protected by a stairrail, the other side of the staircase was a wall. This exposed employees to falls from height.
