29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Jobsite, 147 Lakeview Drive, Honeoye, NY, on or about 5/9/18: The employer did not ensure that procedures were utilized for the control of potentially hazardous energy. An employee was cleaning a Toro concrete mixer and did not shut it off and lock it out prior to reaching through the guard to remove debris. As a result, the employee amputated his left hand middle, ring and pinky fingers.
