29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:  (a)  On or about May 8, 2018, the employer did not ensure that employees were protected from shocks, burns and other electrical hazards from the extension cords used to power the 120V, 13 amp, Ryobi circular saw.  The insulation of the extension cords was cut and broken exposing the live conductors inside.  The extension cords were strung from the free standing building in the back yard of the house being reroofed, to the front yard.
