OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    a) On or about April  14, 2018, the employer did not ensure that employees erecting a 10 x 64 foot wall were protected from struck-by and crush hazards where the wall fell on them while it was being manually lifted and put in place, injuring three workers and resulting in the hospitalization of one worker.. 29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  a) On or about April 14, the employer did not ensure that an accident resulting in an employee hospitalization was reported to OSHA within 24 hours of occurrence.    Abatement note: No abatement certification required.
