29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) On or about 05/14/18, at a 2-story house located at 234 Doat Street, Buffalo, NY: An employee was standing, walking, and working from the aluminum "pic" of a ladder jack scaffold erected in the driveway on the west side of the house. The employee was exposed to a fall of approx. 18 ft. to the ground. No fall protection system was utilized.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.452(k)(5): Scaffold platforms were bridged on to another.  a) On or about 05/14/18, at a 2-story house located at 234 Doat Street, Buffalo, NY:  An employee was standing, walking, and working from the aluminum "pic" of a ladder jack scaffold erected in the driveway on the west side of the house. The employee was exposed to a potential fall or tripping on the aluminum pics because the pics were overlapped (bridged) on the Ladder Jack Scaffolding.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about 05/14/18, at a 2-story house located at 234 Doat Street, Buffalo, NY: An employee engaged in sheeting the steep roof on the west side of the house was exposed to a fall of approx. 20 ft. No fall protection system was utilized. Pitch of the roof was approx.  12 in 12.  NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On or about 05/14/18, at a 2-story house located at 234 Doat Street, Buffalo, NY: Employees were working on the ground doing clean-up work in the vicinity of employees working  above on the roof and on a Ladder Jack Scaffold. The employees were exposed to the potential hazard of being struck by materials that could fall off the roof and scaffold. The employees were not wearing a hardhat.        NO ABATEMENT CERTIFICATION REQUIRED
