29 CFR 1910.179(j)(2)(iii): Frequent Inspection: Monthly inspection of overhead crane load hooks, with a certification record which included the date of inspection, the signature of the person who performed the inspection and an identifier of the hook which was inspected, was not performed. At worksite located at 905 Front Street, Monett, Missouri. Employees using overhead cranes on the production floor, were exposed to a struck by hazard from load hooks. Monthly inspections for deformities or cracks were not accomplished or documented.

29 CFR 1910.179(n)(3)(i): The load was not well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches: At worksite located at 905 Front Street, Monett, Missouri. Employees using overhead cranes on the production floor were exposed to a struck by hazard from lifting loads that are not evenly balanced in the sling or lifting device. Including but not limited to, the Garden Denver brand 4 ton crane Serial Number B4481265 , known to the company as Air Powered Hoist number 04.
