29 CFR 1910.28(b)(1)(i)(C): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by a personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: On or about May 15, 018, the employer failed to ensure that employees were protected from a fall hazard of approximately 8 to 16 feet, by utilizing an adequate fall protection system/device during the cleaning of a residential roof.
