29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) Turner Mountain Construction, Inc. at 300 W Main St. Frisco, CO 80443: On or before 5/15/18 the employer did not protect employees from injury in that employees working beneath where wooden siding was being torn off were not wearing protective helmets. Wood debris with embedded nails was falling directly where employees were clearing the debris. This condition exposed employees to falling objects and puncture hazards.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) Turner Mountain Construction, Inc. at 300 W Main St. Frisco, CO 80443: On or before 5/15/18 the employer did not protect employees from injury in that an employee tearing off siding from inside the basket of an aerial lift was not wearing fall protection. This condition exposed the employee to a fall hazard of approximately 10 feet.
