29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. (a) 6050 East Hinsdale Court, Centennial CO: On or about May 15, 2018, and at times prior, employees were exposed to fall hazards due to lack of a guardrail system, safety net system, or personal fall arrest system while engaged in residential construction activities more than 10 feet above a lower level. During inspection, CSHO observed a single rope hanging next to a pump jack scaffold. CSHO was informed that this rope was the lifeline for two employees while they worked from the pump jack scaffold and while working on a second story roof. The existing personal fall arrest system was insufficient because there were not enough lifelines for each worker, and the lifeline was not secured on the roof. 29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) 6276 Hinsdale Ave, Centennial CO: On or about May 15, 2018, and at times prior, employees were exposed to fall hazards due to working on first story roofs without adequate fall protection measures. One employee was installing a gutter after siding repair had been completed. The employee was standing on first story roofs lacking fall protection measures such as personal fall arrest systems, safety nets, or guardrails. (b) 6296 Hinsdale Ave, Centennial CO: On or about May 15, 2018, and at times prior, employees were exposed to fall hazards due to working on first story roofs without adequate fall protection measures. One employee was cleaning debris off another house after tear-off of siding. The employee was standing on first story roofs lacking fall protection measures such as personal fall arrest systems, safety nets, or guardrails. (c) 6050 East Hinsdale Court, Centennial CO: On or about May 15, 2018, and at times prior, employees were exposed to fall hazards due to working on second story roofs without adequate fall protection measures. Two employees were installing siding on a two story house. There was only one lifeline provided to employees, however both employees were wearing harnesses. Personal fall arrest systems in place were inadequate to protect both employees at the same time.
