29 CFR 1910.179(j)(2)(iii):  Monthly inspections of hooks, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected, were not being performed for the following locations:  Employee(s) engaged in using hooks on hoisting chains to lift materials in the production department located at 16000 W. 108th Street, Lenexa, Kansas were exposed to struck-by hazards due to the hoisting chains and all fastenings and attachments were not inspected for damage or defects as required by a competent person in the following locations:  (a)  Hoist chain in bay 1 was connected to a metal chain sling securing forklift forks, hoist hook had no safety latch.  (b)  Hoist chain hook in bay 3 had a safety latch that was bent upward and not secured against the hook.  (c)  Hoist chain hook in the track cutting bay (south side) had a safety latch that was bent upward and not secured against the hook.  (d)  Hoist chain hook in the track cutting bay (north side) had no safety latch.  (e)  Hoist chain hook in bay 14 had no safety latch.  (f)   Hoist chain hook in bay 18 had no safety latch.

29 CFR 1910.184(d):  Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service:  Employee(s) engaged in using chain slings to lift materials in the production department located at 16000 W. 108th Street, Lenexa, Kansas were exposed to struck-by / crushing hazards due to the chain slings and all fastenings and attachments were not inspected for damage or defects as required by a competent person in the following locations:  (a)  Metal chain sling in bay 1 was lifting forklift forks, the safety latches for the chain sling hook was missing.  (b)  Metal chain sling in bay 3 was missing safety latch on the hooks.  (c)  Metal chain sling in bay 4 had 2 hooks that were missing safety latches.  (d)  Metal chain sling on the floor in bay 7 had missing latch on one of the hooks.  (e)  Metal chain sling in bay 8 had two hooks that were missing safety latches.  (f)  Metal chain sling in bay 11 used to hold up materials had 2 hooks that were missing safety latches.  (g)   Metal chain sling in bay 12 had two hooks that were missing safety latches.  (h)   Metal chain sling in the track cutting bay  (south side) had two hooks that were missing safety latches.  (i)  Metal chain sling being used in bay 14 had two hooks with no safety latches.  (j)  Metal chain sling being used in bay 15 had two hooks with no safety latches.  (k)  Metal chain sling in bay 16 had two hooks missing safety latches

29 CFR 1910.212(a)(3)(iv)(h):  Point of operation guarding was not provided to protect the operator and other employees in the area from hazards such as those created by point of operation, rotating parts, and etc.:    (a) Employee(s) engaged in using a 4-1/2 inch diameter portable right angle grinder to grind metal in Bay 13 located at 16000 W. 108th Street, Lenexa, Kansas were exposed to struck-by hazards due to no lower blade guard being provided.  (b) Employee(s) engaged in using 8 inch diameter portable right angle grinders to grind metal in the production section of the plant located at 16000 W. 108th Street, Lenexa, Kansas were exposed to struck-by hazards due to no lower blade guards being provided.
